Xavier Survey

On 24 April 2006, I announced a survey in lecture.  If you took information on that survey, please read on.  This survey is being run from Xavier University, hence the name of this page.

The survey is done via the URL and password that you received.  Once the survey is completed, you will be mailed a completion form.  You need to print this out and sign it.  You then need to turn in the signed release form and the signed completion form to Peggy Reed in Olsson 223.  You will receive a check for $10 in the mail in a few weeks after that.
